Well see my comment below - if people can opt out but the insurers are obliged to cover anyone on demand, then every single healthy person with economic sense would opt out until the day they got sick. Then opt in and out as they got better or got sick again. Only the sick would ever be 'covered' and there wouldn't be anything like enough in the pot.

Unfortunately there is no way to give universal cover for anything unless you make universal payment mandatory. Ponies for All! means giving up your right to not have a pony if you don't want one and not pay for a pony if you don't want to pay. On the plus side - everyone gets a pony.
